 How to access your Patient Fusion portal


1.  Once you have been invited to join the patient portal, you will receive a registration email from the Patient Fusion email address: healthrecords@patientfusion.com.

2.  Read the email and follow the prompts to sign-up for your Patient Fusion portal account. You will need to know the phone number we have on file for you in order to set up your account.

3.  Create a password and sign-up.

4.  You will then receive a confirmation email. Within that email, click "Confirm my account."

5.  You will be prompted to log-in to your Patient Fusion account in order to begin connecting records.

6.  Choose either "Enter PIN received from the doctor" or "Request security code via phone number."

7. Follow the prompts to finalize access to your record.

Now you are ready to view your health information!

Where can I look online to learn about my health?

Sometimes it feels like you can't trust anything you read on the internet - and oftentimes that is true!  However, there are good places to find accurate medical information online, if you know where to look. Here are some some of Dr. Koretz's favorite websites intended for patient use:
